Job Description

POSITION SUMMARY
Provides outreach and case management to individuals identified by Crisis and Open Access Teams as needing additional support. Works cooperatively with the Crisis Team to respond in person to behavioral health crisis calls to connect individuals in crisis to services, providing crisis stabilization and assistance with coordination of care. The Crisis Team and Open Access Teams are the primary referral sources for this position, but the position also works alongside and assists the 988 MCR Team, CBHL Team, Hospital Diversion Outreach Case Manager, DM3700 Case Manager, and the Court Monitor.

ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
• Provides community-based services to consumer's experiencing a psychiatric or substance related crisis, utilizing independent judgment and decision-making regarding appropriate treatment needed
• Provides active outreach to persons who may be reluctant to engage in services and begin to develop a working alliance towards understanding their individual concerns and needs.
• Completes progress notes in AVATAR within 72 hours of service provided which clearly reflect interventions provided and necessity of those interventions.
• As needed, develops person-centered treatment plans initially and annually, which are strengths based, reflecting consumer's life goals and individualized choices.
• Problem solves independently issues related to housing and housing crisis, finding community resources to assist with basic needs as well as emergency situations, advocate for consumers in regards to receiving proper health care, and be available to problem-solve matters of significance.
• Maintains knowledge of agency technology programs and/or systems needed to do the job and uses them effectively
• Maintains required training and certifications.
• Other duties and/or responsibilities may be assigned by supervisor

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
Education/Certifications:
• Bachelor's Degree in a related field required
• Valid Driver's License and state required vehicle insurance
• CPR/First Aid Certification required

Experience Required:
• 1 to 3 years' experience in a related field required

Required Knowledge:
• Basic knowledge of Microsoft Office Suite (Word, Excel, Power Point).
• Knowledge of behavioral health diagnosis and symptoms.
• Knowledge of community resources and crisis management.
• Prior healthcare technology software experience preferred.

Skills/Abilities:
• Excellent oral and written communication skills.
• Strong interpersonal, communication and collaboration skills.
• Ability to build rapport with consumers struggling to engage in services due to mental health symptoms.
• Ability to make clinical decisions independently.
• Working knowledge of Microsoft Office programs, including Microsoft Outlook, and the ability to use the internet for basic tasks.
• Ability to use basic business equipment.

The noise level in the work environment is usually low to moderate. In addition, the employee will be required to drive to various locations in and around the 3-county catchment area.
